I travelled 4 times in Pearl City Express in Aug-Sep 2013, Twice in SL, once in 3A and once in 2A. I didnt find the coaches/toiltes are unclean. Toilets were cleaned at yard. Infact the bed sheets, pillows provided in AC was super clean.
If Pearl City/KK combo is maintained well then Pandiyan, Rock Fort, Nellai & Pothigai would be better maintained than Pearl City for sure.
The rake was combination of very old coaches, avg and brand new coaches.

ERS-NZM duranto also having 3 tier.. Both are announced in same budget
trains having maintainance at BBQ are worst sir... i have never travelled in north zone train but of south zone, CR is best in coach cleanliness..
Once i accompanied Captain to send him off in GT, which was shunted into platform after cleaning and guess what even the previous trip water bottle was not removed...

I dont have first hand recent experience but when i travelled in Pearl City it was good.
