I think it is just the difference in heights of the source and destination stations. if a train goes from station a 1m altitude to station b 50 m altitude, then it has climbed 50-1=49m
It is height from MSL - Mean Sea Level. Earth level at MSL is taken as Zero. Generally speaking trains climbing from MSL to upward direction, MSL increase. It may be more or less as the train enters plains, mountauns, thar etc.
