Yes, it happened yesterday in “Venkatadri express”, when a passenger suffering from heart attack, fellow passengers tried to pull the chain but train didn’t stopped even after pulling chain in 3 coaches.
Due to the negligence of railway officials he couldn’t get the treatment on time and died with heart attack. Railway officials have to check the functionality of Chains in regular intervals.

The chest pain is usually gripping, like as though a heavy weight is placed on the chest. Sweating, alongwith pain which travels along the left arm, are other symptoms. Good info.
