If it has to be given on the same reservation form, and it can be verbal, I think its forwarded till the clerk only.

No. Clerk absolutely has no role here. He can at the most write down CP option in the form and indicate the same in the system. Final authority lies within the higher authorities and it is finally decided by CRS at the time of Charting.
What's the point of checking the CP option then?
Indeed the final authority rests with the CRS but as far as the forwarding of request is concerned, it is done to the clerk and at the time of charting, which is manual for first AC, the CRS assesses all such requests already made while booking and supervises the allotment as per VIP demand.

Same as submitting a letter without being forwarded/stamped/referred by any higher authority for VIP Quota allotment.
In absence of any recommendation from higher authorities, CRS then looks at the request made by individual passengers.
PS:- If you want to stick to the fact of putting requests to clerks, you may. I am not saying that it is not correct. However, if you may, please understand that the same is just a procedure in name and carries way less weightage and importance in comparison to direct referral from the higher authorities.

Your point taken. A letter to CRS will get more weightage. But I don't get why'd he entertain such requests until the passenger has some contacts on a personal level with him while the clerk is the one who's to deal with such issues? For a layman with no contacts whatsoever, through PRS clerk seems more legal.
