The present position as to upgradation from lower class to the upper class is that the passengers in Waiting List are given accomodation in the upper class. I had recently had an inquiry and I received the following reply.
"The Railways plans to upgrade reserved passengers to the higher classes (if there are vacant berth) through a computerised random selection process undertaken."
"With regard to your mail, we would like to inform you that it is not possible to convert the class from 3AC to 1AC through the net on your request or...

some bloggers here gave instances of double upgradation of instances even though it is assumed that it is one only. (i dont remember the blog numbers, but in a discussion of similar kind, this came out)
i have an instance of my brother in WL(3S) in NDLS BCT Sector getting confirmed and upgraded to 3A from WL of 3S. these things do happen and in cases where there are more vacances in upper class and more wl in lower levels, upgradation may be done two levels.
