Are these normal upper and middle berths or are these Side Upper and Side Middle?
2. PRS did not book bed roll along with ticket, (although I had written Yes for the same on form), will I be able to get in Train??

Side upper/middle/lower will be shown as SUB/SMB/SLB. Here you got two upper berth and one middle, not the side one.
You can get the bedroll from the coach attender by paying 25 rs/berth onboard
