I am updating my experience so that it may be helpful to others in future :)
*First I went to the unreserved ticket counter(UTS) of my railway station and informed the booking clerk that I need a season ticket.

then gave me a form in which I need to fill my personal details like name, father's name, address, phone no, ID card type and no(like voter's ID, PAN, Aadhar,Passport etc), purpose of travel etc. and sign the declaration that we will follow the rules set by railways.
*We need to submit this form, one passport size photograph and a photocopy of the ID card which we mentioned in this form.
*He then gave me an identity card which contains a unique number and pasted my photograph on it and put the rubber stamp of the Chief Booking Supervisor across the photograph. Re.1/-(One Rupee only) will be charged for this identity card and its valid for 7 years.
*He then gave me the monthly season pass by paying the requisite amount. Please note that the unique number of the identity card issued will be mentioned in our season pass. The season pass is valid only if it is produced with the unique ID card issued by railways, otherwise the passenger will be treated as travelling without ticket.
*Also note that we need to put our signature both on the ID card and the season pass,otherwise the passenger will be treated as travelling without ticket.
