Zonal railways issue unique identity cards for visually/mentally/physically challenged persons. Earlier these I cards were issued on the strength of Disability certificates issued by Civil Surgeons/ CMO of the district. I have learnt that the system has been changed and now a railway doctor at zonal headquarters is required to physically verify the persons desirous of having such I cards. Can anyone please lay down the procedure required, formalities and proforma of application form. We run a school for mentally challenged children and it is just physically impossible to move them to our zonal / Divisional Railway headquarters at a distance of 300/150 kms.
