Go to Chief Reservation Supervisor present on your nearest big station with an application mentioning all ticket details & reason + original &xerox copies of Identity card (which proves your relationship like aadhar, voter id, etc.). It should be done before 24 hrs of scheduled departure of train (from origin I guess, not sure)
