Very serious issues highlighted in this news article based on CAG report based on audit conducted in FY 2021-22 for the period from FY 2017-18 to FY 2020-21.
Some glaring ones I haved enclosed screenshots from the article. I am also enclosing the pdf file of CAG audit report. Really decades of negligence towards safety has compounded the situation very badly now.

Yup and also in TOI I think. But more than the news reports which have just covered the summary I think it's better if we go through the full audit report as railfans who love and care for the railways. I just started yesterday and read till page 24 or so will go through the remaining ones after going home in the evening. There were serious lapses on part of the zones in track maintenance activities like TRC inspections, USFD, manpower shortages, welding, idling of track maintenance machines and reasons too listed for the idling, etc. And these issues are across all the zones not just limited to few ones. There are also occasion of zones not following RB directives too like incase of use of FB welding, RT-8527 sleepers.
I thin one senior official also mentioned that in some stretches' there is so much overcapacity i.e more trains running than required that they are not willing to take long blocks for doing maintenance work. Instead half hearted work is done without trying to disrupt the existing traffic which is not the right way to do maintenance.
That's why we need more DFCs in this country. We also need track doubling, tripling projects to be given more priority.
