Friend, The concept of moral responsibility is an emotional one and not scientific one. Resignation on moral grounds is just making oneself a escape goat, and as a result the root cause of accident shall never get eliminated. The new RM shall not be able to stop the accidents.
The person actually responsible for accident need to be penalised, so that there is a fear against committing mistakes and there is no place for carelessness in working.

more... not good and the accident is a direct consequence of his bad/wrong decision. In this case it is not so. RM has given maximum delegation of powers to IR officials like never before.
Right now RM is very disciplined and hard working but his team mates does not appear to be so. RM need to prove how firmly he can deal with the objective of bringing the much needed changes in work culture of IR.
