Partially visible in the pic, 11029 CST Mumbai - SCSMT Kolhapur Koyna Express crossing Kakinada Port - Mumbai LTT Express near Vangani.

Happens to me most of the time. My case, focus will be in compartment of coming train, and loco will be out of focus
When the objects are moving the camera tends to loose focus quite easily. And that is why pics captured onboard mostly looks like this.
So a tip to avoid this,half press your shutter button by pointng out on loco then full press for shutter release
