Yes, one form is sufficient as you will mention the age clearly.
One form is more than enough as long as you are not booking and utilising the LB quota (Senior Citizen Quota).
yes but both of you would be given tickets under General Category. If you are not getting tickets in Gen category but in Senior Citizen category, you may try two different forms to ensure you get at least one confirm ticket for your father.
