I plan to escort a heart patient fitted with pacemaker (age 50+) from Kolkata to Hyderabad. What documents do I need for him to avail travel concession. Where to get the form and who issues it? The man is very poor and all his medical expenses have been taken care of by my family so far. Detailed informatin in this regard will be appreciated. Thanks.

75% in II,SL,First,3A,CC and 50% IN 1AC and 2AC
