Contact PRS with Ticket, ID Proof of both booked passenger & blood related traveler along with written request for change. Once I approached they asked for a notary certificate also to fax to Divisional office. It is a difficult exercise. They advised to try tatkal..

Name can be only changed to your blood relatives i.e only your mother,father,sister,brother,wife,son or daughter. For that photocopy of your ticket,id proof of both the passenger,document proving the relation between the passengers(For ex Ration Card) and an written request and should be submitted to Chief Reservation Officer
