we booked online train tickets for 2 from mysore to kumbakonam under pnr no.4815271097 tr.id 0882799463 2nd ac journey date 22.4.2014 boarding at mysore. due to medical reasons, we have to change the boarding place - bangalore instead of mysore. kindly permit us to board the train at bangalore and do the needful.

Go to the PRS counter and change the boarding stn. Take the Tkt, original ID of the passenger and copy too.

Plz go to the nearest Reservation Counter, with the ticket and ID proof in original, and give request letter for change of Boarding Point from Mysore to Bangalore.
Also please be note that Change of Boarding Point HAS TO BE DONE ATLEAST 24 Hrs, PRIOR to Train's departure from its ORIGINATING STATION and NOT PASSENGER BOARDING POINT.
In your case please change your Boarding Point before 21st April
